Job summary

Would you like to work for our Children's Services in Derby City? (Derbyshire Healthcare NHS Foundation Trust). The service has achieved outstanding by CQC, and we are also accredited with the UNICEF Baby Friendly award.

We are looking for skilled professional Health Visitors who have excellent communication skills and who can deliver compassionate care to support the service, making a real difference to improve the health outcomes for children and families in Derby City.

You will have experience of working with diverse communities and dealing with highly emotive situations, be able to engage with families and build trusting professional relationship. Putting children and young people at the heart of everything you do, you will be passionate about working collaboratively with our partner agencies and will embrace supporting the development of students and new team members.

We are an innovative service providing health visiting services to the families of Derby and we currently have some great opportunities for you to join our team.

Main duties of the job

You will be responsible for the delivery of health visiting services to a defined corporate caseload within Derby City, working within the 0-19 framework ensuring a seamless service to the family as appropriate and working collaboratively with partner agencies.

You will lead on the delivery of the Healthy Child Programme as defined within the service specification, identifying packages of care based on the Universal, Targeted and Specialist models of service delivery. You will provide leadership and supervision to other members of the team and work within Derbyshire Healthcare NHS Foundation Trust policies and procedures.
